    Start with the hospital system, not the premium

    Scripps Health

    Large presence across La Jolla, Encinitas, Chula Vista, and Hillcrest. Scripps has publicly restructured its Medicare Advantage participation in recent years. If your cardiologist or oncologist is at Scripps, confirm plan participation in writing for the coming year before you enroll.

    Sharp HealthCare

    Sharp runs its own health plan and a very large affiliated medical group, and is generally the deepest Advantage participant in the county, with hospitals in Kearny Mesa, Chula Vista, La Mesa, and Coronado.

    UC San Diego Health

    The academic center — Hillcrest, La Jolla, and the Moores Cancer Center. If you need specialized cancer, transplant, or complex neurology care, verify access. Original Medicare plus Medigap guarantees it.

    Kaiser Permanente

    A closed integrated system with its own Advantage plan. Excellent coordination and simple billing if you are comfortable staying entirely inside Kaiser, including its San Diego Medical Center in Kearny Mesa.

    Palomar Health and Tri-City

    The North County inland and coastal safety-net hospitals. Plan networks differ noticeably between North County and the central city — a plan that is strong downtown may be thin in Escondido or Fallbrook.

    Check the doctor, then the group

    In Advantage plans you are usually assigned to a medical group, not just a plan. Ask your physician's office which specific plan and medical group combination keeps you with them for the coming year.

    The enrollment dates that matter

    Initial Enrollment Period

    Seven months around your 65th birthday — three before, your birthday month, and three after. Missing Part B without other creditable coverage creates a permanent 10%-per-year penalty.

    The Medigap six-month window

    The most consequential deadline in Medicare. For six months after your Part B begins, insurers must sell you any Medigap policy regardless of health. After it closes, California allows medical underwriting and you can be declined.

    Annual Enrollment, October 15 – December 7

    Change Advantage plans or Part D drug plans for January 1. Every plan mails an Annual Notice of Change each September — read the two pages listing what changed, especially the drug formulary and the provider network.

    Medicare Advantage Open Enrollment, January 1 – March 31
